Preparing For Your Visit

Please call us in advance for your appointment so that you can be assured of getting the appointments time that is convenient to you. We also ask that you contact us 24 hours in advance to cancel your appointment to prevent additional charges to your account for missed appointments.

 

For new patients to our clinic, you may download a new patient packet and fill in the form, print it and bring the forms with you to save time in the waiting room.

 

All established patients need to sign in each time and fill out this sign in form. To save time during the visit and for your convenience, you may down this form, fill it out, and bring it with you to our office.

 

For annual complete physical examination, please do not eat anything, except for small amount of water, for at least 10 hours before your appointment time.

 

Please bring your medicine with you to our office in each visit so that our physicians know what medicines you are taking. If necessary, in-clinic X-ray and ultrasound exams are available. Other exams, such as MRI, CT, mammogram, stress test, etc. will be scheduled outside of the office.
